Getting a GP in Sweden is not automatic — you have to actively register at a clinic. It takes about 10 minutes online once you have your personnummer, and it's worth doing early rather than scrambling when you are already sick.

The vårdval system

Sweden operates a free-choice primary care system (vårdval) in most regions. This means you are not assigned a clinic based on your postcode — you choose one. You can register at any vårdcentral in your region, whether it is five minutes from your home or on the other side of the city. You can also change your registered clinic at any time, usually with effect from the following month.

Some regions (check your specific region's website) have their own variant of this system with slightly different rules. Stockholm, Västra Götaland, and Skåne all run vårdval. The principles are the same: free choice, same subsidised fee.

What registration actually gives you

Registering at a vårdcentral gives you a named GP (fast läkarkontakt). This doctor becomes responsible for your overall healthcare coordination — tracking chronic conditions, issuing referrals, following up test results. You still see other GPs and nurses at the same clinic for routine issues.

Without registration you can still walk in to any vårdcentral and be seen, but you will not have a consistent care relationship and the clinic has no obligation to prioritise you for routine follow-ups.

How to register — step by step

Online via 1177.se (fastest method):

  1. Go to 1177.se and log in with BankID (you need both personnummer and BankID for this)
  2. Navigate to "Hitta vård" (Find care) and search for vårdcentraler in your area
  3. Each clinic's profile shows which insurance type it accepts, languages spoken, and whether it has capacity for new patients
  4. Select the clinic you want and use the online registration form

Directly at the clinic:

  1. Walk in and ask to register as a new patient (request a "listning")
  2. Bring your personnummer and a valid photo ID (passport or national ID card)
  3. Registration may take effect immediately or at the start of the next month depending on the clinic

By phone:

Call the clinic directly. Numbers are on 1177.se. Some older clinics still prefer phone registration.

What to bring to your first appointment

  • Personnummer (have it memorised or written down)
  • Photo ID
  • List of any current medications, including dosages
  • Previous diagnosis or specialist letters if relevant — Swedish clinics do not automatically receive your medical history from another country
  • EHIC card if you are an EU citizen not yet registered as a Swedish resident

Booking appointments

Most clinics offer three channels:

  • Online via 1177.se — available 24/7, usually the easiest for non-urgent bookings
  • Phone — call the clinic directly; some reserve same-day urgent slots for phone callers only
  • Drop-in — most clinics have drop-in hours for acute minor issues (cuts, ear infections, etc.); check times on the clinic's 1177 profile

By law, regions must offer a same-day telephone contact with a healthcare professional and a GP appointment within 3 days for medical issues that require it. If your clinic cannot meet this, they should direct you elsewhere.

Digital-first clinics

For quick consultations — suspected UTI, minor skin issues, prescription renewals — Sweden has several well-established telemedicine apps:

  • Kry — video consultations in Swedish and English; widely used; can issue prescriptions
  • Min Doktor — similar scope; Swedish-language primary; some English
  • Doktor.se — also offers English consultations

These are connected to the public system in most regions, meaning you pay the standard patient fee (patientavgift), not a private rate. Check the app's fees for your region before using.

These apps do not replace a registered vårdcentral for complex or ongoing care. They are good for one-off issues.

Changing your vårdcentral

You can switch at any time. Log into 1177.se with BankID, go to your registered clinic, and select a new one. The change typically takes effect within a month. Your medical journal stays with the regional system and the new clinic can access it.

The referral system (remiss)

For most specialist care (orthopaedics, dermatology, gynaecology, etc.) you need a referral (remiss) from your GP. Self-referral is possible for some specialties — gynaecology and psychiatry often allow it — but not all. Your GP writes the referral and you are added to the specialist's waitlist.

Waiting times for specialists can be significant (weeks to months). The national vårdgaranti (care guarantee) sets maximum waiting time targets, but they are not always met. Ask your GP for the expected wait and whether there is a private alternative within the public system if the wait is long.

Common problems and fixes

"My clinic is full for new patients." Search 1177.se for nearby alternatives. There is no obligation to register at the closest clinic. In major cities there are usually multiple options within a reasonable area.

"I don't have BankID yet." You cannot register online without BankID. Go directly to the clinic in person. Getting BankID requires a personnummer and a visit to a bank — see the guide on BankID for more detail.

"I moved to a new region." You need to re-register in your new region. Your old registration does not transfer. Your medical journal, however, stays in the national system.

"I need a GP right now and haven't registered anywhere." Call 1177 (the phone line, not the website). The nurse can assess you and direct you to the nearest clinic with same-day capacity or a drop-in service.
